当科における臨床研究の取り組み

概要

臨床研究は日常診療での問題点を解決していく上で重要な方法論であり、当科でも様々な循環器疾患診療の問題解決を目指して行っています。当科の豊富な症例を背景に、冠動脈インターベンション関連のデータベースをはじめ、急性心筋梗塞、不整脈疾患、急性心不全、肺塞栓症など多くのデータベースを当院独自のレジストリーというかたちで構築しており、それを活用して臨床研究を行っております。その成果は、国内学会、国際学会で多数発表しており、最近では医学論文として世界にメッセージを発信しています。

専攻医/専門修練医の臨床研究への関わり

臨床研究には多くのスタッフが関わっていますが、シニアレジデント（専攻医）や専門修練医が関与している機会が多く、以下、シニアレジデントや専攻医の臨床研究へのかかわりの実際について述べます。

まず、専攻医１年目(医師３年目)は、地方会もしくは全国学会において、基本的に症例報告を中心に発表を行い、指導医のサポートを受けながら、プレゼンテーション能力、および個々の症例に対する考察ができるよう力をつけます。その後、専攻医２、３年目(医師４、５年目)は、蓄積された症例を下にデータ収集を行い、適切な統計解析の下、臨床研究データを全国学会、および国際学会で発表できる機会を得ることができます。データ収集はもちろん自分自身で行う必要がありますが、当科秘書さんが患者さんの予後調査などをサポートしてくれます。統計解析に関しては、SPSS、JMPなどの統計解析ソフトを科のパソコンで使用でき、先輩医師の指導のもと、適切な統計解析を行うことができます。高度な統計が必要な場合は年に数回、臨床統計の専門家から直接指導、アドバイスを受けるチャンスもあります。医師６年目以降になると、自分自身で臨床研究を立案するもしくは、多施設共同研究の責任者となり、臨床研究を行います。結果、多数の医師が筆頭著者として論文作成も行っています。
